Neurotransmitter Glutamate is a Novel Neurogenic Initiator in Arthritic Inflammation by K. Westlund High; P0516.1202

چکیده
Over 90% of the population will suffer from arthritis during their lifetime but present treatment regimens cannot completely abrogate chronic episodes of inflammation, pain, or disability. This costs an estimated 60 billion dollars/year in total costs of medicines, disability and lost wages (Arthritis Foundation, 2002). It is firmly accepted that neurogenic factors play a role in peripheral inflammation. There are 18 published case reports of stroke and nerve injury victims demonstrating a sparing of arthritic inflammation and subsequent disease progression on the patients paralyzed side [1]. One case described partial reversal of previous arthritic damage in the paralyzed joint when followed for three years. One case report showed elevated IL-1 levels on both paralyzed and normal sides, but the active arthritic side also had elevated substance P levels and the paralyzed, non-arthritic side did not. To better understand the underlying initiators and arthritic chronicity, studies have been performed to test the hypothesis that neurotransmitter excitatory amino acids (EAA), such as glutamate (GLU), released into the spinal cord and the joint by sensory nerve fiber endings, play a critical role in initiation and persistence of arthritis. Further, GLU contributes directly to inflammatory cascades and the "vicious cycle of inflammation and pain" of arthritis.

Human primary and clonal synovial cells were incubated with glutamate receptor agonists to assess their modulating influence on glutamate receptors N-methyl-d-aspartate (NMDA) NR1 and NR2 and inflammatory cytokines to determine potential for paracrine or autocrine (neurocrine) upregulation of glutamate receptors, as has been shown for bone and chondrocytes. Clonal SW982 synoviocytes constitutiv...

Activation of ionotropic glutamate receptors has been shown previously to be essential for the development of secondary thermal hyperalgesia. The present study assessed involvement of group I metabotropic glutamate receptors (mGlu) in both the induction and maintenance phases of secondary thermal hyperalgesia initiated by knee joint inflammation in rats. The dose dependence of each drug in anta...

متن کاملJoint inflammation is reduced by dorsal rhizotomy and not by sympathectomy or spinal cord transection.
OBJECTIVES To investigate the role of primary afferents, sympathetic postganglionic efferents and descending systems on the central control of peripheral inflammation. METHODS Acute inflammation was induced by intra-articular injection of kaolin and carrageenan into the knee joint cavity of the rat. Before the induction of the arthritis, a unilateral dorsal rhizotomy, a chemical (phentolamine...
متن کاملRole of neurogenic inflammation in pancreatitis and pancreatic pain.
Pain arising from pancreatic diseases can become chronic and difficult to treat. There is a paucity of knowledge regarding the mechanisms that sensitize neural pathways that transmit noxious information from visceral organs. In this review, neurogenic inflammation is presented as a possible amplifier of the noxious signal from peripheral organs including the pancreas. The nerve pathways that tr...
